Preschool Admission Physical Exam & Health Form Service

Preparing your child for childcare, preschool, or kindergarten? Most schools and early childhood centers in Hong Kong require a certified medical examination form signed by a registered medical practitioner prior to admission.

At Seed Medical, we make your child's pre-school health check quick, stress-free, and thorough. Our registered doctors will review your child's medical history, evaluate their immunization records, perform necessary basic physical checks, and complete your school's required form accurately and efficiently.

What is Included in the Standard Service?

Standard Service Fee: HK$ 400

Our standard package covers the basic health evaluation typically required for general school enrollment:

Medical Record Review: Evaluation of your child's background health information provided by parents
Immunization Check: Review of your child’s vaccination record / immunization card (Immunization Record / "Blue Book")
Basic Physical Assessment: Age-appropriate physical screening (e.g., height, weight, basic vital signs, and physical wellness checks as requested by the school form)
Form Completion: Medical review, official registration signature by a registered doctor, and official clinic stamping for one (1) school form

Note: Specific physical examination items depend on the individual school's form requirements, your child’s age, and the doctor’s clinical judgment.

To help us complete your form promptly, please ensure your child attends in person accompanied by a parent or legal guardian, and bring:

In-Person Evaluation Required: The child must be physically present at the clinic for the assessment.

Doctor’s Clinical Discretion: Health evaluation sections must be completed solely by the examining doctor based on actual clinical findings.

Turnaround Time: Same-day form completion is common when all records are provided and no additional lab tests are required. However, same-day completion cannot be guaranteed if health records are incomplete or further tests are required.

Admission Outcome: The health check provides an objective medical assessment and does not guarantee school admission.

General Observation: Basic observations during the visit do not constitute a comprehensive neurodevelopmental or speech assessment.
1. What does the HK$400 standard service include?

It includes a review of your child's health and immunization records, a basic physical assessment tailored to the school form, and the completion, signing, and stamping of one (1) official school admission physical examination form by a registered doctor.

2. Does my child need to fast before the appointment?

No fasting is needed for standard pre-school physical exams. If your school form explicitly requires blood work that necessitates fasting, our clinic staff will advise you in advance after reviewing your form.

3. Can I get the completed health report on the same day?

In most standard cases where all documents/vaccination records are complete and no external lab tests are required, the form can be completed on the same day. However, final timing depends on doctor availability and clinical findings.

4. What if my school form requires a vision or hearing test?

Standard physical checks include general clinical observation. If your school requires formal audiometric (hearing) or optometric (vision) diagnostic testing, these specialized services can be arranged separately upon request.

5. What if I have forms for multiple schools?

The standard fee of HK$400 covers the completion of one (1) physical exam form. Additional forms for other schools can be completed for a supplementary fee during the same visit.
